Re:  SCADR Forms 102 and 104


ADMINISTRATIVE ORDER


Pursuant to the provisions of S.C. Const. Art. V § 4,

IT IS ORDERED that the Notice of ADR (SCADR Form 102), bearing a revision date of 11/2012, and the Proof of ADR or Exemption (SCADR Form 104), bearing a revision date of 11/2012, are approved for use in the Circuit Courts of South Carolina. These forms replace SCADR Form 102 bearing a revision date of May 2007 and SCADR Form 104 bearing a revision date of May 2007.

The forms have been revised to replace references to the Standing Order for Alternative Dispute Resolution with the Alternative Dispute Resolution Rules which now govern Alternative Dispute Resolution in South Carolina.

The forms shall be available on the South Carolina Judicial Department website at www.sccourts.org under the ‘Forms’ link.
