Re: Uniform Adult Guardianship and Protective Proceedings Jurisdiction Act
ORDER
Pursuant to the provisions of S. C. CONST. Art. V, § 4,
I find that due to the implementation of the Uniform Adult Guardianship and Protective Proceedings Jurisdiction Act and upon recommendation of the Probate Judges Advisory Committee, revisions to certain uniform probate court forms and the development of the following new forms are necessary. Therefore, effective as of the date of this order, the eight (8) forms listed below are approved for use in the Probate Courts. Four (4) of the forms listed (530GC, 540GC, 575GC, and 578GC) are mandated. Mandated forms that are computer generated must include all language in its entirety as it appears in the approved form. All other uniform probate forms listed below are designated as discretionary. The 2013 revised forms (530GC and 540GC) supersede all previous versions of these forms.
In addition, all probate court forms relating to Guardianships and Conservatorships and referenced with PC will be modified to reference GC.

These mandatory forms will be available on the South Carolina Judicial Department website at www.sccourts.org located under the Probate Court Forms link.
