Pursuant to the provisions of S.C. Const. Art. V § 4,

SCCA Form 234, Civil Action Coversheet, bearing a revision date of 02/2018, is approved for use in the Circuit Courts of the State of South Carolina. The form has been updated to include a Nature of Action Code for Interpleader (690), a Nature of Action Code for Incapacitated Adult Settlement (790), and a notation that states the Form is not required to be filed in E-Filed Cases.

This form is available on the South Carolina Judicial Department website at www.sccourts.org/forms.
