RE: Family Court Forms
ADMINISTRATIVE ORDER
Pursuant to the provisions of Article V, Section 4 of the South Carolina Constitution, the following family court forms, all bearing a revision date of (10/2020) are hereby amended effective October 21, 2020:
- SCCA434 Rule 24 – Clerk’s Affidavit
- SCCA436 Notice of Support Enforcement Hearing
- SCCA441 Order Terminating Child Support Based on Emancipation
- SCCA455 Order to Pay Support through Court
Rule 24 – Clerk’s Affidavit (SCCA434) is amended to include additional details regarding date, time, and address of personal service.
Notice of Support Enforcement Hearing (SCCA436) is amended to include informational language to the Plaintiff regarding service upon the Defendant.
Order Terminating Child Support Based on Emancipation (SCCA441) is amended to clarify the date upon which child support obligations may cease.
Order to Pay Support through Court (SCCA455) is amended to include the State Disbursement Unit’s mailing address for child support payments.
These forms are approved for use in family courts of South Carolina and supersede all previous versions effective October 21, 2020. These forms will be available on the South Carolina Judicial Branch website at http://www.sccourts.org/forms.
