Re: Civil Action Coversheet SCCA/234

ORDER

Pursuant to the provisions of S.C. Const. Art. V § 4,

The Civil Action Coversheet, SCCA/234, bearing a revision date of 03/2009 is approved for use in the Circuit Courts of the State of South Carolina.  The coversheet has been amended to have a nature of action code for Transfer of Structured Settlement Payment Rights Applications. The nature of action code will be 760.

The Civil Action Coversheet is required with all initial pleadings filed in the Court of Common Pleas.  This coversheet should be completed, in its entirety, by the attorney filing the action and served on the defendant with the Summons and Complaint. This coversheet shall remain as an attachment in order to document the nature of the action that is being filed and as proof of payment of the filing fee.  Form SCCA/234 (03/2009) supersedes and replaces the coversheet bearing a revision date of 04/2008.

The civil coversheet is available on the South Carolina Judicial Department website at www.sccourts.org.
