Re: Civil Action Coversheet SCCA/234

ORDER

Pursuant to the provisions of S.C. Const. Art. V � 4,

The Civil Action Coversheet, SCCA/234, bearing a revision date of 07/2009 is approved for use in the Circuit Courts of the State of South Carolina.� The coversheet has been amended to include a line for the original Notice of Intent to File Suit case number under the �Notice/ File Med Mal (230)� nature of action option which is located in the� Torts/Professional Malpractice section of the coversheet.� When the complaint�s nature of action is medical malpractice, the plaintiff should indicate the Notice of Intent to File Suit action case number on the line provided on the Civil Action Coversheet.�

The Civil Action Coversheet is required with all initial pleadings filed in the Court of Common Pleas.� This coversheet should be completed, in its entirety, by the attorney filing the action and served on the defendant with the Summons and Complaint. This coversheet shall remain as an attachment in order to document the nature of the action that is being filed and as proof of payment of the filing fee.  Form SCCA/234 (07/2009) supersedes and replaces the coversheet bearing a revision date of 03/2009.

The civil coversheet is available on the South Carolina Judicial Department website at www.sccourts.org.�
