Re: Civil Action Coversheet SCCA/234
ORDER
Pursuant to the provisions of S.C. Const. Art. V § 4,
The Civil Action Coversheet, SCCA/234, bearing a revision date of 11/2009 is approved for use in the Circuit Courts of the State of South Carolina. The nature of action code for Administrative Law Court appeals to Circuit Court has been removed to conform to 2006 SC Laws Act 387 which provides that Administrative Law Court decisions be appealed to the Court of Appeals.
The Civil Action Coversheet is required with all initial pleadings filed in the Court of Common Pleas. This coversheet should be completed, in its entirety, by the attorney filing the action and served on the defendant with the Summons and Complaint. This coversheet shall remain as an attachment in order to document the nature of the action that is being filed and as proof of payment of the filing fee. Form SCCA/234 (11/2009) supersedes and replaces the coversheet bearing a revision date of 10/2009.
The civil coversheet is available on the South Carolina Judicial Department website at www.sccourts.org.
